APPOINTMENTS:

For GP appointment bookings please visit Health Engine website or give us a call at the Reception via 08 9452 2055. Appointment can not be made via email. Please inform our receptionist if you have a number of issues, so we can book a longer consult for you.

NEW PATIENTS:

Prime Medical Centre welcomes new pateints. Please arrive earlier to your appointment to fill up the new patient registration form. Please bring Valid Medicare Card and Photo ID. If you have one of the following cards, please bring along: (Pensioner Concession Card, Health Care Card, Commonwealth Seniors Health Card, DVA Card)

MEDICAL CERTIFICATES/ REPEAT PRESCRIPTIONS/ REFERRAL LETTERS:

You will need to make an appointment with your doctor either face to face or telephone consult for a repeat prescriptions, medical certificates and referral letters. This will ensure good medical practice and that our doctors meet their medico-legal obligations to patients and you condition is being optimally managed.

TEST RESULTS:

Test results should be followed up by an appointment. The clinic nurse or receptionist will contact anyone with results that need urgent attention.

CANCELLATION POLICY:

If you can not attend your booked appointment, please call us at least 2 hours before your appointment time to cancel or re-schedule appointment. Otherwise, you will be charged a missed appointment fee of $30.

FEE POLICY:

PRIME MEDICAL CENTRE IS MIXED BILLING PRACTICE !

Patients who will be eligible for BULK BILL are:
  1. 1. Children 16 years and younger
  2. 2. Adults 65 years and older
  3. 3. Pension/Concession Card holders
  4. 4. Health Card Card holders
  5. 5. DVA Card holders
  6. 6. GP Management Plan, Team Care Arrangement Plan and Reviews
  7. 7. Health Assessments

  8. *All other patients will be MIXED BILL.

CONSULTATION FEES:
Standard consult (Less than 15 mins) : $70 ----- Out-of-pocket fee: $28.60
Long consult (15 to 30 mins) : $120 ----- Out-of-pocket fee: $39.90

TELEPHONE CONSULTATION FEES:
Standard consult : $60 ----- PRIVATE BILLING !

AFTER HOURS:
Standard consult (Less than 15 mins) : $85 ----- Out-of-pocket fee: $31.10
Long consult (15 to 30 mins): $135 ----- Out-of-pocket fee: $ 42.55
After Hours: Saturday - 1pm onwards. Sunday and Public Holidays - Whole day

Mental Health Care Plan/ Review (out of pocket fee) ---- $45.00 - $35.00

Payment is required in full at the time of consultation. We accept cash, all major credits and EFTOPS.
Note: Mental Health Care Plan/Review, Follow up, Results, Recall and Script Renewal appointments are NOT Bulk Bill !!!

  • Worker's compensation related consults - (Patients need to pay privately until they receive the claim number!)
  • Work related health assessment i.e. Pre-employment medical assessment
  • Fitness for return to work and duty assessment
  • Driver Licence Medical (Private and Commercial)
  • Dangerous Good Licence
  • HUET Medical Check
  • Insurance Medical reports
  • Lawyer Medical reports
  • Non Medicare claimable paperwork etc.
  • * Certain procedures may attract out-of-pocket fees, so please check with the receptionist for FEE SCHEDULE !
  • * Gap-fee may be applicable for Allied Health appointments, so please check with the receptionist.
  • * Telephone consults are not available if you have not attended our Medical Centre in the last 12 months.
  • * PLEASE cancel your appointment three hours in advance, if you can not make it, otherwise, non-cancellation fees 30$ will apply.

HOME VISIT:

Our doctors are willing to do home visits during surgery hour, offered at a time convenient to the doctor. Please be aware home visits are not completely covered by medicare and there is a private consultation fee payable on the day. Please ask our receptionist for advice when making an appointment.

CULTURALLY APPROPRIATE CARE:

Our practice provides culturally appropriate care to patients. Many of our doctors speak language other than English. We have GP's that speak Bangali, Burmese, Cantonese, Hakka, Hokkien, Hindi, Mandarin, Malay, Punjabi etc.

PRIVACY:

Pirme Medical Centre is committed to protecting our patients' privacy according to the Privacy Act. Access to computer records containing your information is restricted to medical staff and password protected, with all hard copies of patient documents handle in a secure manner. A hard copy of our complete privacy policy is available on request at the reception.

PATIENTS RIGHTS & RESPONSIBILITIES:

MY HEALTH RECORD:

My Health Record is a secure online summary of your health information. You can control what goes into it, and who is allowed to access it. You can shoose to share your health information with your doctors, hospitals and other healthcare providers. Please visit www.myhealthrecord.gov.au

IMMUNISATION:

The Australian Childhood Immunisation Regeister (ACIR) records information about immunisations given to children under the age of 7 years who live in Australia. Children under 7 years of age enrolled in Medicare are automatically included on the ACIR. If your child is not enrolled in Medicare they can be added when your doctor or health care provider sends the details of their immunisation to the ACIR. You can request a statement of immunisation at any time by visiting the ACIR website or by calling the ACIR on 1800 653 809 (free call).

Prime Medical Centre is a fully accredited, privately owned general practice. We aims to provide the highest standard of patient care in a friendly and welcoming environment. We are passionate about setting standards of excellence in our practice in the provision of excellent health care for the community.

The practice is located at the Forest Lakes Shopping Centre. We are a mixed billing practice, open 7 days a week. We have a few GP available, male and female.

We also have an onsite Pathology - Australian Clinical Labs, Clinical Psychologist, Chiropractor and Dietitian. Please enquire at reception for more details.
